Some examples include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medication errors<\/li>\n\n\n\n<li>Falls during care<\/li>\n\n\n\n<li>Racism and\/or discrimination<\/li>\n\n\n\n<li>Delayed care<\/li>\n<\/ul>\n\n\n\n<p><strong>Patients share concerns<\/strong> about things like:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Communication with health care providers<\/li>\n\n\n\n<li>How much autonomy or involvement they have in planning their care<\/li>\n\n\n\n<li>The accessibility of health care sites and programs<\/li>\n<\/ul>\n\n\n\n<p>The consequences of heath care-related harm and concerns can be far reaching and may impact how safe patients and communities feel seeking health care in the future.<\/p>\n\n\n\n<p>The ministry is reviewing how health care-related harm is addressed by two separate but related quality review processes in B.C.&#8217;s public health care system:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>internal patient safety reviews (initiated by health care providers)<\/li>\n\n\n\n<li>patient care quality reviews (initiated by patients or their representatives).<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Internal patient safety reviews<\/h2>\n\n\n\n<p>When something goes wrong, health care providers review what happened to understand why it occurred and how to prevent it in the future. Currently, legal restrictions allow providers to speak openly during these reviews without their discussions being used in court. However, these protections can limit what information is shared with patients and families.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Patient care quality reviews<\/h2>\n\n\n\n<p>Each health authority has a Patient Care Quality Office where patients or someone they trust can share concerns about their health care and receive a response. If concerns are not resolved, they can be sent to the Patient Care Quality Review Board (PCQRB) for independent review. The PCQRB may then make recommendations for resolution and quality improvement to health authorities and\/or the Minister of Health.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>As part of this work, the ministry is seeking public feedback to improve the transparency, accessibility, responsiveness, effectiveness, and cultural safety of quality review processes. Your voice matters. What you share will help inform health care policy and improve how care is delivered, so it better reflects the needs of patients and communities.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Equity, diversity and inclusion<\/h2>\n\n\n\n<p>To create review processes that are safe, easy to access and able to create real change, we need to hear from communities who may be more affected by harm in health care or who may face barriers when submitting a concern or taking part in a patient care quality review. This includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Racialized communities<\/li>\n\n\n\n<li>2SLGBTQIA+ community members<\/li>\n\n\n\n<li>People living with disabilities<\/li>\n\n\n\n<li>Those living in rural or remote locations<\/li>\n\n\n\n<li>People living with mental health challenges<\/li>\n\n\n\n<li>People who have accessed B.C. Correctional Health Services<\/li>\n<\/ul>\n\n\n\n<p>To help us centre the voices and experiences of these communities, public survey respondents will be asked to self-identify where they are comfortable doing so.
